Each video you upload and caption uses one video from your plan, whatever its length (up to your plan's limit). Fixing words before the captions are burned in doesn't use another one.
No. If a video fails to process, the credit is returned automatically.
No. Your allowance resets each period: every 2 weeks on Free, monthly on Basic and Pro.
